Passport Family Resource Guide

Please note that on June 21, 2010 two plain language guides for the Passport program, will be posted on the MCSS website. One of the guides is for families and caregivers and one is for individuals with a developmental disability.

The Passport Resource Guides were developed based on recommendations from the evaluation of the Passport program that was conducted by Deloitte in 2007/2008. The evaluation found that families needed more information about funding options, possible uses of their Passport funding, the development of personal plans, and contact information for their local Passport designated agencies.

The Ontario Government is changing its system of services for adults with a developmental disability
to make it fair, accessible and sustainable.

One of the key changes is a new application process so people will know where to go to apply for
services and supports, the same steps will be used to apply for services, and the same rules will be
followed when decisions are made regardless of where in Ontario a person lives.
We want to hear what you think about how such a regional application entity should function in the
Northern Region (Note: the Northern Region is made up of the districts of Sudbury, Manitoulin,
Algoma, Thunder Bay, Kenora and Rainy River).

Please join us on Tuesday, November 10 from 1:00 to 3:30 pm at the Lion’s Den (upstairs),
Nemi Recreation Centre, Hwy. 6, Little Current
to hear about the changes the government is
making, and to give us your feedback.

Service Coordination helps men and women, who have a developmental disability, access and coordinate services in the Districts of Sudbury and Manitoulin. Our services are offered in both English and French.

Service Coordination provides:

  • Central access and coordination within the Adult Developmental Service System
  • Individual centered service that enables the person and their caregivers to achieve and maintain the highest level of independence
  • Ongoing needs assessment and planning with the individual and this may involve natural networks, health, social services, education and the legal systems
  • Maintenance of one waiting list for the adult developmental service system
  • Maintenance of a database of information on the current and future needs of the people registered and the services being accessed within the Adult Developmental Service System

The YWCA Sudbury Service Coordination provides a central system to process all requests for admission to the following Adult Developmental Service System agencies:

  • Sudbury Developmental Services
  • Community Living Greater Sudbury
  • Sudbury Community Service Centre – Case Management
  • L’Arche Sudbury Inc.
  • Christian Horizons – Sudbury
  • Northeastern Mental Health Centre – Developmental Clinical Services
  • Wikweminkong Anishinabe Association for Community Living
  • Community Living Manitoulin
  • Community Living Espanola
  • Nadmadwin Mental Health Clinic – Community Living Centre
  • Chapleau Health Services – Supported Independent Living

How does it work?

  1. Families and individuals contact us when they need assistance to meet a need. Individuals can receive services from us by referring themselves or by being referred by someone else.
  2. When we receive a referral, it is assigned to a Service Coordinator.
  3. The Service Coordinator contacts the person wanting services and also contacts other people in their lives. Together they determine the person’s needs.
  4. The Service Coordinator and the individual develop a plan to meet the needs by considering all services in the Developmental Service System and the community. Applications could be made to services, individuals could be placed on a waiting list, planning meetings could be held.
  5. Agencies in the Adult Developmental Service System inform Service Coordination when they have vacancies in their residential and day programs. The Service Coordinator can apply for respite, psychology and social work services. The other Developmental Service System supports can be accessed immediately when an individual’s needs are a priority.
  6. Once an individual is receiving service from an agency(ies), Service Coordination can be re-involved when there are changes

Why can I not go directly to the agency?

Service Coordination’s role is to assess your needs with you and determine what services you need. This is the only way to get on a waiting list.

Does Service Coordination make accessing services harder?

At one time each of the nine agencies that provide services in the Districts of Sudbury and Manitoulin did their own intake. When you needed a service, you had to knock on several doors and tell your story many times. With the introduction of Service Coordination, you only need to tell your story once and you can access a service that is available, or your name is placed on a waiting list for all nine agencies.

Why is Service Coordination a part of the YWCA?

Service Coordination is a part of the YWCA Sudbury for the following reasons:

  • We have provided services to people with a developmental disability since 1978
  • We only provide Service Coordination so we can avoid conflicts of interest
  • We have a means to serve clients across both Districts of Sudbury and Manitoulin
  • We are equally accessible to all clients including those in rural area
  • We are sensitive to the multi-cultural/linguistic needs of our client population

My son lives in a Group Home, so why do I not hear from my Service Coordinator?

When you are accessing services from other agencies in the Sudbury Manitoulin Adult Developmental Service System you have a main worker. This person plans with you for what you need. Your Service Coordinator would get involved if you or your worker have a concern or want a change of services.

Why do I need to be registered with Service Coordination when I do not need any service?

There are two main reasons why it is important to be registered with Service Coordination:

  1. If your needs change there is already someone who knows you so you do not need to go through an extensive needs assessment process.
  2. We gather aggregate information that does not identify who you are, but it provides our system with information available to plan for services. This information helps us predict what services you might need in the future.

I have been on the waiting list for a group home for a long time. What can I do for my daughter in the meantime?

Try to teach her some skills like cooking, washing her hair, going to the bank, eating without assistance, going to the washroom and any other skill that will help her become more independent. In the meantime, your Service Coordinator can help you access the services and supports that you need and that are available.

How does Service Coordination and access to services work?

  • You can refer yourself or have someone refer you to Service Coordination.
  • Your Service Coordinator will assess your needs and plan with you to meet your needs.
  • You and your Service Coordinator will apply for available services and supports that you need from the Sudbury Manitoulin Adult Developmental Service System or from other agencies in the community.
  • Your Service Coordinator will help determine if you have friends and family that can help you.
  • If the service you need is not available you will be placed on a waiting list and your situation will be rated.
  • The agencies tell us when they have vacancies and can serve someone.
  • When this happens Service Coordination will ask them to serve someone with many major needs and have a high rating (like someone without a place to live). If the agency cannot meet the needs of this high rated person, then a profile describing someone who wants the service but is doing okay without it is given to the agency for consideration for the vacancy.
  • The agency decides who it can serve.
  • When your needs change contact your Service Coordinator and let her know.
  • You can be referred to Service Coordination when you are already receiving a service from the Adult Developmental Service System, by asking your worker to make a referral. Otherwise, if you are not a receiver of these services you can contact us directly.
  • Service Coordination maintains one waiting list for the Developmental Service System and prioritizes/ranks needs.
  • Service Coordination plans for people who are not involved in the system, to prevent the need for services or to determine if their needs have changed.
  • It maintains a database of information and identifies problems in the system.
  • It is not an emergency response system.
  • Your Service Coordinator will not take you to doctors’ appointments or provide you with a group home at the YWCA Sudbury.
