Passport Family Resource Guide
Please note that on June 21, 2010 two plain language guides for the Passport program, will be posted on the MCSS website. One of the guides is for families and caregivers and one is for individuals with a developmental disability.
The Passport Resource Guides were developed based on recommendations from the evaluation of the Passport program that was conducted by Deloitte in 2007/2008. The evaluation found that families needed more information about funding options, possible uses of their Passport funding, the development of personal plans, and contact information for their local Passport designated agencies.

The Ontario Government is changing its system of services for adults with a developmental disability
to make it fair, accessible and sustainable.
One of the key changes is a new application process so people will know where to go to apply for
services and supports, the same steps will be used to apply for services, and the same rules will be
followed when decisions are made regardless of where in Ontario a person lives.
We want to hear what you think about how such a regional application entity should function in the
Northern Region (Note: the Northern Region is made up of the districts of Sudbury, Manitoulin,
Algoma, Thunder Bay, Kenora and Rainy River).
Please join us on Tuesday, November 10 from 1:00 to 3:30 pm at the Lion’s Den (upstairs),
Nemi Recreation Centre, Hwy. 6, Little Current to hear about the changes the government is
making, and to give us your feedback.

564-6699 ext. 511 in Sudbury and toll free 1-800-265-1222 ext. 511 before the meeting date to
confirm your attendance. If your attendance is not confirmed, we may be unable to accommodate

Service Coordination helps men and women, who have a developmental disability, access and coordinate services in the Districts of Sudbury and Manitoulin. Our services are offered in both English and French.
Service Coordination provides:
- Central access and coordination within the Adult Developmental Service System
- Individual centered service that enables the person and their caregivers to achieve and maintain the highest level of independence
- Ongoing needs assessment and planning with the individual and this may involve natural networks, health, social services, education and the legal systems
- Maintenance of one waiting list for the adult developmental service system
- Maintenance of a database of information on the current and future needs of the people registered and the services being accessed within the Adult Developmental Service System
The YWCA Sudbury Service Coordination provides a central system to process all requests for admission to the following Adult Developmental Service System agencies:
- Sudbury Developmental Services
- Community Living Greater Sudbury
- Sudbury Community Service Centre – Case Management
- L’Arche Sudbury Inc.
- Christian Horizons – Sudbury
- Northeastern Mental Health Centre – Developmental Clinical Services
- Wikweminkong Anishinabe Association for Community Living
- Community Living Manitoulin
- Community Living Espanola
- Nadmadwin Mental Health Clinic – Community Living Centre
- Chapleau Health Services – Supported Independent Living
How does it work?
- Families and individuals contact us when they need assistance to meet a need. Individuals can receive services from us by referring themselves or by being referred by someone else.
- When we receive a referral, it is assigned to a Service Coordinator.
- The Service Coordinator contacts the person wanting services and also contacts other people in their lives. Together they determine the person’s needs.
- The Service Coordinator and the individual develop a plan to meet the needs by considering all services in the Developmental Service System and the community. Applications could be made to services, individuals could be placed on a waiting list, planning meetings could be held.
- Agencies in the Adult Developmental Service System inform Service Coordination when they have vacancies in their residential and day programs. The Service Coordinator can apply for respite, psychology and social work services. The other Developmental Service System supports can be accessed immediately when an individual’s needs are a priority.
- Once an individual is receiving service from an agency(ies), Service Coordination can be re-involved when there are changes
